SimSun;">2019年，浙江省委省政府做出推进新时代大救助体系建设部署，全面推进“1+8+X”大救助体系建设。目前，以信息系统为支撑，初步建立了由政府各救助部门和社会力量参与的大救助格局，构建了由五个层次四大类型组成的分层分类的社会救助制度体系：救助对象上分为特困、低保、低保边缘、支出型贫困救助、临时救助等5个层次，功能作用上分为基本生活救助、专项社会救助、急难社会救助和补充性救助4大类型。对不同层次的救助对象，根据需求分别给予不同类型救助。同时，建立健全精准识贫、主动发现、长效帮扶、诚信评价和探访关爱等五项工作机制，确保体系建设落细落实。</span></p><p SimSun;">为进一步提升救助效率、整合救助资源，避免以往民政与扶贫部门在对象识别上交叉认定、动态管理上期限不同等问题导致的行政资源浪费，2020年起，浙江省按照“识别标准统一、认定部门统一、对象管理统一、救助帮扶统筹”的思路，由民政部门统一负责低保对象、特困人员、低保边缘对象等困难人群的认定识别和动态调整，其中，农村地区低保、低保边缘、特困三类救助对象即为扶贫对象。民政部门牵头建立统一的信息系统和数据库，对救助对象进行动态管理，并与扶贫等部门进行大数据比对，确保对象认定精准。相关部门在工作中发现未纳入保障范围的困难对象，通过大救助信息系统协助办理救助申请，民政部门进行线上审核确认。</span></p><p style="text-align:justify;letter-spacing: 1px;line-height: 2em;margin-bottom: 15px;"><strong><span style="font-family: 宋体, SimSun;">二、瞄准靶向，监测指标科学化、精准化</span></strong></p><p SimSun;">结合省情实际，瞄准因病、因学、因残、因失业等主要返贫致贫因素，设立了患病住院、患有慢性病和重大疾病、残疾人登记办证、新增失业、高考录取学生等5项预警指标，并赋予一定权重，通过监测预警量化救助需求程度。协调人社、教育、卫健、医保、残联等部门，通过线上线下相结合的形式获取信息数据，每月与监测对象身份数据进行交叉比对。根据指标数据与对象数据身份信息重合叠加情况量化打分，将得分情况按区间划分，由低到高确定蓝色、黄色、橙色、红色四个预警级别，叠加的困难越多，预警级别越高，救助需求越迫切。</span></p><p SimSun;">为加快预警信息的反馈速度，提升基层工作效率，真正做到“让信息多跑路、让群众少跑路”，我省研发了困难群众动态管理监测预警系统，预警信息通过系统直接反馈至市、县民政部门和乡镇（街道）。基层根据预警信息不同等级的时限要求，及时组织人员开展入户核查，对符合条件的家庭和人员给予相应救助。通过分级预警、分类管理、限时核查，极大提升了预警工作和救助工作的时效性。</span></p><p SimSun;">根据监测系统实时发出提醒信息，基层工作人员主动上门调查核实家庭情况，对符合低保条件的家庭和人员纳入保障范围；对突发急难性、临时性、紧迫性事故导致困难的家庭给予临时救助，并将入户调查及办理情况通过系统进行反馈，做到闭环管理。对超期未完成入户和反馈结果的预警信息，系统将自动向相关县区推送催办信息，同步向省、市系统管理员推送逾期提醒消息，便于省市民政部门督促相关县区加快工作进程。同时，根据人员类型，定期向扶贫、人社等行业部门推送扶贫对象、失业人员预警信息，由相关部门落实相应帮扶措施，通过综合施策，有效防止困难群众返贫致贫。截至目前，全省共发出预警信息526349条，其中红色45600条，橙色25874条，黄色245200条，蓝色209675条，累计对31373名符合条件的困难群众落实救助政策，有27075人纳入低保范围，对4298人实施临时救助，部分对象落实了两种以上的救助措施。</span></p><p SimSun;">按照“大救助、大统筹”理念，从“病、残、老、幼”入手，稳妥有序推进政策扩围。特困供养政策范围扩展至有部分收入但收入低于特困供养基本生活保障标准的“二无”对象；低保政策范围扩展至家庭收入在2倍低保标准以内的重病重残人员，全面实施“单人保”；专项救助政策范围扩展至家庭收入在2倍低保标准以内的23种大病患者，给予自负医疗费用75%的年度专项救助，并对因病支出型困难家庭开展生活救助；临时救助政策范围扩展至非户籍的急难对象，以及无收入来源的农民工等未参保人员。逐步构建完善覆盖特困人员、低保对象、低保边缘家庭、支出型困难家庭、急难对象等5个圈层、44类困难人员的弱有所扶大救助体系。截至2020年底，全市以政策扩围方式纳入救助的困难对象占比超过45%。</span></p><p SimSun;">去年9月，苏州市由民政部门牵头，联合10部门发文建立“低收入家庭登记制度”，统一开展全口径的低收入家庭认定工作。强化部门协同、政策衔接，推动认定结果在部门间共享互认。民政部门认定的低收入家庭，就业、教育、住房、医疗等10类专项救助业务直接适用，认定有效期为一年。认定工作主要通过“三个一批”方式展开，即直接认定一批。对于在册“单人保”家庭直接认定为低收入家庭；申请审核一批。通过政策宣讲、走访慰问，并同步开通线上线下申请渠道，引导和发动居民申请；主动发现一批。通过大数据监测、网格化巡访、“铁脚板”入户，瞄准近年退保和病残家庭主动帮办认定，发现“沉默少数”。截至目前，全市共认定低收入家庭2235户5210人。今年春节，所有低收入家庭均享受春节慰问，家庭成员中的学生全部纳入教育资助范围，医疗自负费用6000元以上的患者全部享受了年度专项医疗救助。</span></p><p
